By creating an account with the Local Civil Registrar Office of Kawit, Cavite Appointment and Document Request Management System, you agree to the following:
You confirm that the personal details and identification document you submit are true and accurate, and understand that false information may result in account rejection or termination.
Information you provide is used solely to process appointments and civil registry document requests, verify your identity, and send status notifications via SMS/email, in line with the Data Privacy Act of 2012 (RA 10173).
You are responsible for keeping your password and security question answers confidential. The office will never ask for your password by phone, email, or SMS.
Approval of appointments and release of documents remain subject to verification of submitted requirements by office staff, in accordance with existing civil registry regulations.
